The question and decision is very dependent on who the web master is. If he or she is unexperienced with optimizing a server, I will always recommend going with a managed server. Let the host set you up and control the settings for you.

I'd say unmanaged if you have knowledge of system administration and managed if you don't.
I always go unmanaged but it's because I have some knowledge in the field and I am not afraid of searching for solutions online if things go wrong. Of course, if you don't want problems, go with unmanaged and make sure that you do not pay for a "fake managed" service!
